Where Do I Get A Birth, Death Or Marriage Certificate?

You can request certified copies of births, deaths, or marriages from Pequannock township’s Vital Statistics and Records Division. Records can be obtained in person or by mail. The event must have occurred within the Township of Pequannock to obtain the record.

Hours of Operation

Monday through Friday (excluding Holidays)
8:30 a.m. to 4:30 p.m.; 

Extended hours to 7:30 p.m. the second Tuesday of each month and 8:30 a.m through 1:00 p.m. the following Friday.

To Request in Person

Certificate obtained same day (typically about five minute wait).

  • Identification is required (No Exceptions)
    If requesting on behalf of a child, spouse, sibling or parent additional identification is required. You must prove relationship to the individual of record (birth certificate or marriage certificate along with driver’s license or passport)
  • $25 fee for first document; $5 for each additional certified copy of the same record. (Cash or Money Order)
  • Completed application at Health Department.

Certificates typically processed and mailed same day (takes about 1 to 2 weeks).
